These are the only forms that will be accepted at certification.

When completing the Registration form your child's "LEGAL NAME" must match the name printed on their birth certificate. Remember to include middle names. Children must also sign the Parental/Guardian Permission and Waiver and include their middle name. Please verify that your child has printed and signed their middle names  

Medical History form should be completed by Parent/Guardian. Please fill out the entire form. If your child has specific needs that the Coach should be made aware of please include that information as well.

Physical Form must be completed in its entirety ONLY by a Licensed State Examiner (medical doctor, physician assistant, nurse practitioner, etc.) This form must be submitted PRIOR to the athlete participating in Pop Warner (THIS INCLUDES PRACTICES). No other forms are acceptable.

Both Player and Parent must fill out this form, be certain the form is singed by both the participant and parent.

Registration, medical and Physical forms completed,

and report cards (no copies) showing all 4 quarters to their

head coaches no later than August 1st!

Children are ineligible to participate until all paper work has been turned in and verified.
